Process of OPD Consultation / Investigation in ESIC
- Visit the ESIC counter of Sanjivani Hospital for OPD consultation / investigation under ESIC scheme.
- Show your ESIC card, ESIC referral, and ID proof (Prefer Aadhaar card) at the ESIC counter.
- Receive your consultation and diagnostic slip from the dedicated counter.
Process for IPD Admission in ESIC
For Normal IPD Admission: Visit the ESIC counter of the hospital and provide your:
- ESIC Card & Online ESIC Referral Form
- ID Proof (Aadhaar Card preferred)
- Doctor Note / Relevant Investigation slip
- For dependent family members, PAN card or Passbook copy is required
For Emergency IPD Admission: In life-threatening emergency cases, all you need is your ESIC card and Aadhaar card for immediate admission in the Emergency ward. Remaining documents can be submitted after admission within the prescribed time interval.
